Life Treatment Centers accepts the following managed care plans:

• Anthem BCBS Indiana
• CareSource Indiana
• MDWise Indiana
• Managed Health Services (MHS) Indiana

We also accept Traditional Indiana Medicaid.

Life Treatment Centers accepts Recovery Works referrals for Outpatient Treatment and Level IV Recovery Residencies.

For private insurance, only Anthem BCBS Indiana is accepted.

A Self-Pay option is available and operates on a sliding fee scale.

Please note: Insurance covers psychotherapy services only and does not include housing.
