FULL TIME: PRIMARY TREATMENT COORDINATOR

SCHEDULE

1ST SHIFT (8AM-4PM WITH TWO SHIFTS 12:00P – 8:00P) WITH FLEXIBILITY FOR EVENING CLASSES. THERE IS A ROTATING ON-CALL FOR CRISIS CALLS THAT ROTATES BETWEEN ALL COORDINATORS/COUNSELORS.

PAY

$45,000 WITH A MASTER’S DEGREE BUT WILL GO UP BASED ON LICENSE.

Requirements

  • Master’s Degree in Mental Health Counseling, Addictions Counseling, Marriage and Family, Psychology, or Social Work and eligible for state license, preferred but individuals in the above field Master’s Programs required.
  • Active LSCW, LSW, LMHC, LMHCa, LCAC, LAC, LMFT, or Psychology License in Indiana, preferred.
  • Valid Driver’s License

Job Description

We are currently looking for an energetic and a self-starter to join our team as a Full Time Primary Treatment Coordinator. Similar positions include: counselor, LSW, LCSW, LCAC, LCACa, LAC, LMHC, LMHCa, LMFT, therapist. This is a great position to help people in our community reach the needed resources to be successful in recovery.

Counsel and aid persons served and their families requiring assistance dealing with substance abuse problems through interviews, group sessions, treatment planning, reviewing of records, and counseling.

Essential Functions:

  • Interview persons served, review records and communicate with other clinical staff and professionals to evaluate the condition of the person served.
  • Formulates plan for treatment and rehabilitation of person served.
  • Counsel person served individually and in group sessions to assist person served in maintaining abstinence from alcohol and/or drug dependency.
  • Counsels family members to assist family in dealing with and providing support for person served.
  • Refers person served to other support services as needed such as medical evaluation and treatment, social services, and employment services.
  • Monitors condition of person served to evaluate success of therapy and adapts treatment as needed.
  • Prepares, dictates, and maintains reports and clinical records as required in a timely manner.
  • Prepares documents for presentation in court and accompanies person served to court as needed.
  • Serves as a member of the clinical team.
  • Attend meetings and training as needed.
  • Take on-call responsibilities as needed.
  • Maintain professional conduct.
  • Treat persons served with dignity, respect and free from: abuse, neglect and exploitation.
  • Report suspected or observed abuse, neglect, or exploitation.
  • Complete all required paperwork/computer work accurately in a timely manner.
  • Report to work as scheduled.
  • Other duties as assigned.

RESIDENTIAL TECHNICIAN

SHIFTS AVAILABLE:

  • 1st shift 7am-3pm
  • 2nd 3pm-11pm
  • 3rd 11pm-7am
  • Floater- will be available for any shift as needed
  • Weekend availability

  • Maintain professional conduct
  • Complete a Post-Offer Drug Screen and Criminal Background Check

Life Treatment Centers offer a residential program comprised of continuing care, outpatient therapy, substance use prevention and education, and after care. We are currently searching for positive, self-movitated, and dedicated Part-Time Residential Technicians to support our work.

  • Ensure the individuals in the residential program are abiding by the rules of the program
  • Conduct urine drug screens, oral swabs, and alcohol breath tests
  • Document behaviors in the Electronic Medical Record System (EMR)
  • Conduct orientation for residents new to the residential housing program
  • Take and record temperature, pulse and respiration rate for residents experiencing illness
  • Ensure self-administration of medication and maintain required records
  • Monitor residents at all time
  • Complete all required paperwork/records accurately and in a timely manner
  • Other duties as assigned

COOK

SHIFTS AVAILABLE:

  • M-F 9am – 5:30 pm
    Occasional weekend as needed

Requirements of the position:

  • Associate’s degree (A.A.) or equivalent from two-year College or technical school preferred, but not required
  • 2 years experience cooking in a restaurant or cafeteria setting
  • Due to the individuals we serve and being a substance abuse treatment facility, there are certain things we cannot hire for and we do conduct criminal backgrounds
  • Due to being a substance abuse treatment facility a pre employment drug screen will be conducted
  • Serv Safe certification

Job Description

  • Cooks food products in quantities according to the menu and number of persons to be served.
  • Cuts vegetables and fruits when available and serves.
  • Bakes cakes and makes puddings when product is available.
  • Prepares side dishes (Cole Slaw, Macaroni Salad, Jello, Etc.) utilizing the products that are available.
  • Plans nutritious menus appropriate for persons in recovery, taking advantage of foods in season and local availability.
  • Utilizes the food donated by the USDA Fish & Loaves program. (i.e. Corn, blueberries, and tomatoes, etc)
  • Serves meals at the appropriate times.
  • Create menus and meal plans as required by regulatory agencies, grants, etc.
  • Directs activities of workers who assist in preparing and serving meals.
  • Washes dishes, cleans kitchen and kitchen equipment in accordance with the required kitchen clean up duties form and the St. Joseph County Health Department regulations.
  • Ensures that the dining area is restocked and cleaned.
  • Order supplies and food
  • Prepares requisitions of food and supplies for the supervisor, to maintain the daily operations of the kitchen.
  • Maintain professional conduct.
  • Treat persons served with dignity, respect and free from: abuse, neglect and exploitation.

  • Complete all required paperwork/computer work/records accurately and in a timely manner.
  • Assist with food pick ups and restocking

  • Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form

FULLTIME INTAKE COUNSELOR

SHIFTS AVAILABLE:

  • M-F 8 Hour Shifts

Requirements of the position:

  • Valid Driver’s License
  • QBHP designation: Master’s Degree in Mental Health Counseling, Marriage and Family, Psychology, Addictions Counseling, or Social Work and eligible for state licensure preferred or working on the above-mentioned masters, required.
  • Professional License designation: LCSW, LCAC, LMHC, LMFT, preferred but not required.
  • Proficient with computers as 90% is data entry.

Job Description

  • Perform all needed documentation during the intake/admit process with the person served – approx. 30 – 60 minutes.
  • Scanning, uploading, etc.
  • Data entry in external tracking systems.
  • Medication Verification and Count
  • Conduct prescreening, jail acceptance letters, call-backs
  • Scheduling appointments
  • Assisting with Medicaid Insurance Enrollment
  • Prepare and maintain reports and records in accordance with agency policies and procedures.
  • Participate in agency committees.
  • Interview persons served, review records and communicate with other clinical staff and professionals.
  • Initiate a person-centered treatment plan.
  • Provide individual counseling, if needed.
  • Prepare, facilitate, and document treatment groups, including skills and development training.
  • Participate in weekly staffing/meetings to discuss progress.
  • Counsel family members to assist family in dealing with and providing support for person served.
  • Refer person served to other support services as needed such as medical, mental health, social services, and employment services.
  • Prepare, dictates, and maintains reports and clinical records.
  • Prepare and communicate documents to legal referral sources, courts, etc.
  • Attend meetings and training as needed.
  • Participate in rotation for on-call phone crisis intervention.
  • Maintain professional conduct.
  • Treat persons served with dignity, respect and free from: abuse, neglect, and exploitation.
  • Report suspected or observed abuse, neglect, and exploitation.
  • Complete all required paperwork/computer work/etc. accurately and in a timely manner.
  • Report to work as scheduled.
  • Other duties as assigned.

Knowledge and/or Abilities

  • Knowledge of substance abuse
  • Ability to solve practical problems and deal with a variety of concrete variables.
  • Ability to follow written, oral, diagram, or scheduled instructions.
  • Ability to relate well to persons served, family members of persons served, co-worker, supervisors, customers, etc. while maintaining a level of professionalism.
  • Ability to solve practical problems and deal with a variety of concrete variables in a situation where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form
  • Professional License designation: LCSW, LCAC, LMHC, LMFT, preferred but not required.
  • Proficient with computers as 90% is data entry.
  • Due to this position’s location temperatures and noise levels may fluctuate.
  • Will be supervised by a licensed professional or QBHP.
  • This position may face stressful situations, as this position is fast paced requires answering phone calls, talking with potential persons served, and referral sources.
  • Expected travel is 0 – 25%.
