CASE MANAGEMENT

Our Clinical Case Managers are a fundamental part of the LTC treatment team. They provide direct assistance in gaining access to needed medical, social, educational/vocational, and other services. Their whole person approach ensures that all identified needs are addressed to ensure sustained change.
